The objectives of the tummy tuck are the flattening and temper of the abdomen giving the appearance of an athletic abdomen, drying the excess skin reducing or eliminating stretch marks with scars located within the bikini line so that they are barely visible, improve the definition of the waist, decrease the thickness of subcutaneous fat around the abdomen and correct the relaxation the abdominal wall.

Am I a candidate for a tummy tuck?

Normally women seek this surgery after the pregnancies when the skin has become very flaccid and / or with multiple stretch marks. Usually it is recommended to defer this procedure until having more children is not desired, because the skin and muscles can relax again.

It is very important that the patient is emotionally stable, in a small percentage of patients, depression after surgery may be experienced and if there is a previous emotional problem, this period can become a serious illness.

When is a tummy tuck performed?

The tummy tuck is usually performed when a person is dissatisfied with the shape of the abdomen. Usually done when women have had pregnancies which have left flaccid skin or with multiple stretch marks and who do not want any more children. The reason is because a new pregnancy will cause again a relaxation of the abdominal wall and the skin and possibly the development of more stretch marks. The miniabdominoplastia is a good alternative in case on which the patient wishes to improve her abdomen but still is not clear or is projected to have more children.

How do I know if I need tummy tuck or if a liposuction is enough?

Liposuction is an excellent alternative to improve the abdominal contour when the abdomen skin is of good quality, ie, it does not have much flaccidity and stretch marks and has good elasticity. When the skin is very flaccid and only a liposuction is performed generally the results are unsatisfactory, the skin is often left with multiple irregularities that are difficult to correct.

If the flaccidity is located only in the lower abdomen is an excellent alternative is the miniabdominoplastia.

How is an abdominoplasty performed?

The abdominoplasty is usually performed under regional anesthesia.

The surgery is designed according to the specific needs and characteristics of each patient.

It’s generally recommended to start the surgery with an abdomen liposuction to reduce subcutaneous fat, a transverse incision in the lower abdomen part is made, so that the scar is located within the bikini line, the tissues are dissected over the muscles until the upper part of the abdomen, then the muscles are taken towards the center improving the waist, the skin is stretched and excesses are dried.

In the advanced skin an orifice is made to relocate the navel in its normal position. Finally drains coming out above the pubic bone are placed and the wounds are closed.

What techniques of abdominoplasty exist?

There are different techniques of abdominoplasty, the main difference is usually the direction and location of the scars. It is important to discuss with your surgeon, in advance, the shape and location of scars. Some surgeons make the incision completely horizontal, others use a “V” or “w” shape and others use an incision which follows the bikini line.

I always use a design which follows the bikini line so the scar is perfectly camouflaged with the underwear. Obviously the incision varies according to the type of underwear that the patient uses.

There are also significant differences in the design of the skin cut and the direction in which the latter tightens. I use the design which allows to better define the waistline than conventional abdominoplasty.

You can walk immediately the day after surgery, in fact it is recommended to walk at least 3 or 4 times a day for 1 to 2 minutes each time. After the fourth day it’s recommended that you try to walk upright, usually the sixth or seventh day you should be able to walk with your back completely straight.

What are the risks of the abdominoplasty?

The abdominoplasty is a safe procedure when performed by a qualified plastic surgeon and on a healthy patient. However there are certain risks that you must know:

Abnormal scars: With a proper technique in most patients the scars are of excellent quality and perfectly camouflage within the bikini line.

Occasionally by factors specific to the patient enlargement, thickening or dark pigmentation of the scar may occur which may warrant correction. Also a narrowing the navel may occur.

Infection: It’s not frequent. It may occur in 1 to 3% of the cases. It can cause the wound to open.

Necrosis or death of the skin: It occurs in 0.1 to 5% of cases. It is more common in smokers.

It is due to the lack of proper circulation in any segment of skin. Without good circulation the skin dies and becomes a wound which may take time to heal. If the segment of the necrotic skin is large, it may warrant resection of that devitalized tissue. Once surrounding tissue is healthy the closing of the wound may be made.

When this complication is presented it requires a lot of patience from the patient and the surgeon, it’s a process which treatment warrants usually a long time. Usually the end result is satisfactory despite the distress and inconvenience twhich this complication causes.

Pulmonary embolism: It’s a complication which can occur in any prolonged surgery.

It occurs in 0.8% to 1.1% of patients undergoing abdominoplasty.

It is due to the formation of clots in the leg veins and their subsequent release into the lungs in a sudden manner causing respiratory difficulty and may compromise the patient’s life.

For their prevention it’s very important the use of pneumatic compression systems in the legs and early ambulation. This is why we strictly use Flowtron pneumatic compression stockings in all our patients and prophylaxis with low molecular weight heparin.

Is it possible to remove stretch marks left after the pregnancy with a tummy tuck?

The stretch marks that are located below the navel and are accompanied by marked flaccidity of the skin can be removed during a tummy tuck. If there is no significant flaccidity in the abdominal skin, the efforts and costs of performing a tummy tuck is not justified just to remove stretch marks, since stretch marks are replaced by a scar. Stretch marks located above the navel are not removed during surgery.
